Its occurrence in the oral cavity is extremely rare due to limited presence of smooth muscle in this region. We present a case of leiomyosarcoma of the mandible, possibly originated from intraosseous region, in a 90−year−old female who exhibited the swelling and tooth mobility. Imaging showed a radiolucent lesion, and biopsy confirmed leiomyosarcoma through histopathological findings and immunohistochemical staining positive for vimentin, SMA, and HHF−35. Additionally, we present a brief literature of 32 case reports on mandible leiomyosarcoma in this article, which revealed a broad age range (7−67 years), a slight female predominance, variable tumor sizes (1−18 cm), and frequent mandibular involvement.
