| 内容記述 | 本研究の目的は，認知症高齢者の便秘に対して看護師がどのようにアセスメントして援助方法を決定しているのかを明らかにすることである．看護師11名を対象に半構造化面接を行った結果，【いつもと違う様子から便秘の仮説を立てる】【本当に出ているか複数の指標から判断する】【便秘の原因を考える】【症状出現の原因を考え便秘以外を消去する】【援助方法を決めるために‘排泄歴’を把握する】【便の降り具合から浣腸・摘便・下剤を選択する】【自然排便の援助をする】【いつもと違う様子からすぐに便を出し予想通りだったと確認する】など14カテゴリが生成された．認知症高齢者の便秘に対する看護師のアセスメントの特徴として，“いつもと違う”に注目すること，対応に緊急性があると判断した場合，すぐに排便の援助を実施し後付けで［便秘の確信］を得ることが示唆された． | |||||||||

| 内容記述 | This study was performed to clarify what assessments prompted nurses to determine the need for excretion support for elderly dementia patients with constipation. A semistructured interview of 11 nurses resulted in the preparation of 14 categories, including “predict constipation when an unusual appearance is observed in a patient,” “use several indexes to judge excretion,” “identify the cause of constipation,” “solving problems other than the constipation itself in order to identify the cause of symptoms, ” “understand the concept of excretion to determine a support method,” “predict the accumulation of stool based on the amount of food consumed to determine a support method,” “select an enema, disimpaction, or a purgative drug depending on the stool location in the bowel,” “support natural excretion,” and “take measures to manage excretion when unusual symptoms are observed if there are no other causes and confirm whether or not the prediction is correct.” The characteristics of nurses’ assessments of constipation in elderly dementia patients suggest that they should pay close attention to “any unusual appearance,” and when the situation is deemed urgent, they should immediately help the patient defecate and thereby achieve a sense of confidence in overcoming constipation in the future. | |||||||||
