| 内容記述 | This paper reports a case of upper airway obstruction occurring after inter-maxillary fixation and wire aspiration after an urgent release of the fixation. A 26-year-old female with a diagnosis of mandibular prognathism was subjected to sagittal split ramus osteotomy and inter-maxillary fixation under general anesthesia. The inter-maxillary fixation was carried out at several sites using metal ligature wire. After the extubation, vomiting reflexes and paradoxical breathing occurred, and then there was an urgent release of inter-maxillary fixation. There was obvious edema in the pharynx region and continuous hemorrhaging from the wound, and re-intubation and respiratory treatment was administered. Afterwards, a wire-like image was observed in the dexter bronchus division of the chest radiograph. Thereafter, the wire image moved to the intestine region, and it was excreted naturally. This experience suggests the necessity of a reexamination for postoperative air duct management including inter-maxillary fixation. | |||||
